*Position Title: *Doula/Family Health Educator *Department: *Care & Population Health Management *Reports To: *Population Health Manager *Status: *Full Time, Non-Exempt *Hours: *Monday – Friday *Supervision: *This position does not supervise any other staff *Date Updated: *October 17, 2023 *Summary: *This position focuses on improving the health status and social determinants of health (SDOH) of PrimeWest Health members during the prenatal and postpartum period, both individually and in groups.
The Doula/Family Health Educator is primarily responsible for working in communities with members who benefit the most from their services.
This position is a trusted frontline health worker who is knowledgeable in prenatal and postpartum care, labor and delivery, and the care of healthy newborns.
The Doula/Family Health Educator bridges cultural and linguistic barriers, assists with access to care, addresses SDOH, and improves health outcomes for pregnant members and babies.
As a critical link between communities and the Care & Population Health Management team, the Doula/Family Health Educator works to reduce health inequities, boost cultural competence, and empower individuals and communities to achieve healthy pregnancies through advocacy, education, and support.
This position plays an integral part in reducing emergency room use and unplanned hospitalizations; increasing access to holistic, integrated health care; and connecting members to covered services.
The Doula/Family Health Educator works closely with obstetric, medical, and behavioral health providers; health care facilities; care coordinators; county partners; and other agencies to improve member care and outcomes.
The Doula/Family Health Educator travels by car throughout PrimeWest Health’s member counties in order to carry out some job responsibilities.

Reporting Accountability* This position is responsible for meeting the following regulatory, contractual, or other requirements on behalf of PrimeWest Health: 1.
Knowledge of Minnesota Department of Human Services (DHS), Centers for Medicare & Medicaid Services (CMS), and National Committee for Quality Assurance (NCQA) requirements 2.
Knowledge of the DHS Families and Children program and contract.
Specific reporting responsibilities include the following: a.
Public Health goals b.
Low birth weight report c.
Annual analysis of PrimeWest Health’s maternal/child program including pre- and postnatal care, depression screening, infant immunizations, member incentives, high-risk deliveries, and American Indian/Tribal involvement.
d.
Annual analysis of the Nurse-Family Partnership program *B.
Role and Key Responsibilities* Assist the Population Health Manager, Women & Children Care Coordinator, and PrimeWest Health staff in the following as needed: 1.
*Clinical/Professional Functions* a.
Under the direction of the Population Health Manager and the Women & Children Care Coordinator, collaborate with members’ care teams to provide education and support to pregnant and postpartum members b.
Meet in person with members at their home and provider/community services offices c.
Plan and implement pregnancy health education programs d.
Assist in the development of population-based programs targeting high-risk groups and individuals, implementing programs through group and individual counseling and education e.
Assume a key role in implementing and maintaining the comprehensive care management program for pregnant and postpartum members f.
Assist in developing and implementing quality improvement plans aimed at improving the health of mothers and infants g.
Assist as necessary with American Indian/Tribal health issues related to pregnancy and childbirth h.
Staff appropriate committees as assigned *2.
Training and Education* a.
Conduct prenatal education with community partners, providers, and member groups b.
Attend community events and workshops, providing education and information about healthy pregnancy, childbirth, infant care, the importance of prenatal and postpartum visits, and the PrimeWest Health Focused Wellness Pregnancy Program c.
Lead focus groups surrounding pregnancy and postpartum care d.
Develop and maintain provider education materials related to maternal/child health activities and programs e.
Assist with provider training related to covered services for women and children.
This includes county and/or health care home (HCH)/Accountable Rural Community Health (ARCH) program training.
f.
Conduct individual clinic visits in collaboration with county Child and Teen Checkup (C&TC) coordinators; Women, Infants, and Children (WIC) coordinators; and PrimeWest Health Provider Services staff g.
Assist with developing and maintaining portions of the PrimeWest Health website relating to maternal and child health h.
Review and modify applicable policies and procedures related to women and children *3.
Collaboration with External Entities* a.
Collaborate with county agencies to achieve community health improvement goals if required b.
Collaborate with county Public Health agencies regarding lead screening, C&TCs, immunizations, prenatal health, violence prevention, tobacco cessation during pregnancy, obesity during pregnancy, WIC clinics, and encouraging recommended prenatal and postnatal visits and screenings c.
Conduct Public Health workgroups if necessary d.
Give presentations, conduct telephone assessments, and participate in external meetings and committees *C.
General PrimeWest Health Responsibilities* 1.

Educational Requirements and Required Qualifications* 1.
Licensed practical nurse (LPN) diploma or certified/seeking certification as a doula 2.
Bachelor’s degree in a health-related area preferred 3.
Demonstrated knowledge of, and proficiency in, Word, Excel, and Outlook required.
Experience in relational databases and business information systems preferred.

At least 3 years of experience working in prenatal/postpartum care 5.
Experience providing community/group education preferred 6.
Flexibility and ability to handle multiple tasks.
Demonstrated organizational skills and attention to detail.
Ability to work effectively with a variety of people both within and outside of the organization.
Excellent verbal and written communication skills.

Ability to work well under pressure in a fast-paced environment.

Ability to sit at a desk or computer station most of the time.
Ability to occasionally lift and/or carry such articles as file folders and small boxes up to 30 pounds.

Ability to travel approximately 50 percent of the time, including occasional overnight travel.

Appropriate Licensure and Certification Requirements* 1.
Must possess one of the following: a.
Current and unrestricted LPN license b.
Current and unrestricted doula certificate or be actively pursuing this certificate 2.
Must attend continuing education sessions annually to support license requirement 3.
Must possess current and unrestricted driver’s license *G.
